Reasons one and two: wrong version, missing prerequisites

Not the official version. Four are always refused: your own photocopy, a printed scan, a company-stamped certified true copy, and a document already outside the recipient's recency window. None carries the signature and seal the DFA verifies. Civil registry documents must be the PSA version; local registrar copies usually do not qualify. Fix: order the official version; the document itself is fine.

Private document missing notarisation or the court certificate. Powers of attorney, affidavits and board resolutions need notarisation plus a Certificate of Authority for a Notarial Act from the supervising Regional Trial Court. Notarising and then missing the CANA is the most easily overlooked return reason. Fix: supply the missing step; the notarised document does not need re-signing.

Academic records have their own version problem: taking a school certificate straight to apostillisation without the education regulator's certification is the leading return reason in that category.

Reasons three to five: direction, exclusions, destination

Reversed direction. The DFA FAQ is explicit: documents for use inside the Philippines cannot be submitted, and the Office of Consular Affairs does not authenticate foreign documents. Fix: go back to the issuing country.

Excluded by the Convention. Documents executed by diplomatic or consular agents, and administrative documents dealing directly with commercial or customs operations, fall outside it — these may still need the traditional consular route even for convention states.

Wrong destination assessment. Two things decide it: whether the destination is a convention state, and whether the document is a type the Convention covers. Finland and Germany are convention states but objected to the Philippines' accession, so documents to and from those two still require consular legalisation.

Reason six: the document itself is wrong

Misspelled names, wrong dates, mismatched parents' names, two documents that disagree. This is the expensive one, because the fix is not on the authentication line — it means a correction at the issuing agency, then a fresh official version, then the whole chain again.

Apostillisation does not correct content. It certifies the signature, the seal and the signatory's capacity, and nothing else. Most people discover the error at the receiving end, across a border and a time zone.

A pre-filing checklist:

1. Is this the issuing agency's official version?
2. For private documents, are both notarisation and the court CANA in hand?
3. Is the direction right — Philippine documents only?
4. Is the destination a convention state, and is the document a covered type?
5. Do names, dates and parents' names match your other documents?
6. Is the underlying document still inside the recipient's recency window (often 3 or 6 months)?
7. Is the appointment booked? Counters have been appointment-only since 1 December 2024, and re-filing needs a new booking.
