Visitor extensions accumulate by duration, not by tally. Many short extensions can leave you far from the ceiling; two long ones can bring you close to it.

The three real ceilings
Cumulative stay. Continuous stay as a visitor is capped, with different brackets for visa-required and visa-free nationalities, counted from the most recent recorded entry. This is the hard wall.
The bracket available this time. Options and conditions are whatever is currently in force, and the range for a first extension often differs from later ones. Choose by how certain your next few months are, not by which looks cheaper.
How long the status can survive. A work visa depends on employment, a student visa on enrolment, marriage-based residence on marital status. When the underlying relationship ends, extension stops being a question of duration.

Four statuses, four mechanics
Visitor (9A): no count limit, only a duration ceiling. Crossing roughly 59 days brings in the registration card; leaving after 6 continuous months brings in the ECC.
